I agree with some other folks here that following the AO3 model would work well! Include a trigger warning (or the more neutral “content note”) for a few of the very most common/obvious triggers/squicks, plus maybe a “Choose Not to Warn” option. People could also warn for/note additional stuff that’s not in the list at their discretion.
